
2009/2010 Registration Information 
Times are changing and at ISMS, so is our registration process. Registration for the 2009/2010 school year at will be held Wednesday, August 5, 2009 from 1–7 p.m.
In the past, an information packet has been mailed out prior to registration. We will not be sending these packets home this year. Instead, there will be a link through the ISMS website for the Student Health Information sheet. Please print this page, complete it, give it to the nurse during registration. Student schedules, a movie permission form and an extra-curricular information card (required for all students) will be in folders that will be given to you during registration. Please have your child return these important forms to his/her homeroom on the first day of school.
Prior to registration (June-July 31), we are asking parents to visit the ISMS website to view their child’s information currently entered in Harmony. You will need to log in using your child’s USERNAME and PASSWORD. It is very important that you review this information to verify that it is correct, even if you do not have any changes. If you do not have access to a computer beforehand, you will be asked to review this information when you come for registration. Again, you will need your child’s login and password to access this information at that time.
Book Fees, Lunch Money, Etc. 
During registration, you will have the opportunity to put money into your child’s accounts. Checks will need to be made out separately for the following items:

Athletics Forms 
Any student wishing to participate in athletics during the 2009/2010 school year needs a physical (dated after May 1, 2009) on file with ISMS before trying out for the sport in which they are interested.
Health Forms 
Indian Springs Middle School will be continuing our Tylenol program for the 2009/2010 school year in which you may elect to participate. This year we will also be adding Ibuprofen to your options. In this program, we are offering an opportunity for your child to receive TYLENOL/Acetaminophen (325 mg) and/or IBUPROFEN/Advil (200 mg) throughout the school year. The cost will be - Tylenol - $1.00 for 50 tablets and Ibuprofen - $1.00 for 25 tablets. A permission form needs to be on file to participate.
Volunteer Form 
Throughout the school year there are many opportunities for parents to come into ISMS and help with a variety of things including field trips, the ISMS annual fish fry, Reality Store, and Career Day. In order to participate in these events, you will need to have a Volunteer Authorization and Release Form on file with the school.
Progress Reports & Report Cards 
The 2009/2010 school year will also bring some changes to the progress report and report card procedures at ISMS. In the past, copies of your child’s reports have been sent home with the students. In an effort to strive towards a paperless environment, beginning in September 2009, progress reports and report cards will be sent home electronically. A parent may request a hard copy but the school will only print one if requested. Please contact the 7th grade office if you would like to receive a hardcopy of either document.
School Pictures/Health Screenings 
School pictures will be held on Thursday, August 20, 2009. Health screening will also be done that day. Vision screening will be done for 8th grade and hearing screening will be done for 7th grade.
Back to School Night 
ISMS Back to School Night will be held Thursday, August 20, 2009 from 7:00-8:00 p.m . This is a good opportunity to meet one on one with your child’s core teachers, administrators and guidance counselors. This evening has been set aside for parents only.
Band/Choir – Drop/Add 
All 7th and 8th grade students enrolled in band and/or choir for the 2008/2009 school year will automatically be registered for band and/or choir for the 2009-2010 school year unless a written request from a parent is received. Changes to the band and choir schedules may only be made the first week of each semester of the new school year and must be in writing. If you have any questions concerning this process, please feel free to call your grade level counselor, administrator or the 7th grade office.
Activities 
Any student who did not pass the math or language arts portions of the spring ISTEP+ test will be placed in a math or language arts remediation class during activity time. If your child did not pass either math or language arts, they will be placed in math remediation. If your child wishes to be in band and/or choir, and they have not passed these portions of ISTEP+, a parent/administrator meeting must be held before your child can be removed from the remediation classes. You may contact your child’s administrator or grade-level office if you have any questions concerning this issue or you wish to set up a meeting.
After School Clubs
Indian Springs is pleased to announce several new clubs for students starting November 3, 2009. Please see below for these exciting opportunities that are available to your child(ren). All clubs (except the ART Club) are open to any interested student in grades 6, 7 and 8, but each club is limited to the first twenty students who enroll.
If you are interested in signing up your child for one or more of these clubs, please fill out and return the permission slip. A new permission slip/note will be required to attend each club meeting. All permission slips should be turned in to the receptionist prior to each club meeting.
